Background: A crazy group of Arabs living in a New York slum are against the sexual exploitation of women at Hip-Hop concerts. Eminem is playing this evening. The Arabs have broken into the Concert Hall and have killed Eminems Bodyguards and taken his sexy dancers hostage. They want to put an end to the show before it begins.
Counter-Terrorists: Eminem will be arriving in a few hours. A hole has been blown through a wall in the backstage area. Rush in and rescue his dancers, and settle the situation, before the show is to begin.
Terrorists: Prevent the Counter-Terrorist
force from rescuing the dancers at any cost.

Background: This is what happens when good guys live in bad neighborhoods. This slummy area is the home of the famous, partially-crippled, Boogie Pianist, Markus Friedrich. Some hostile crack-addicted Terrorists from the homeless-haven, Hondo's Crackhouse, need more DOPE! NOW!!! They wanted to take Mr. Friedrich's Grand Piano to the pawn shop to get more dope!! Unfortunately, the piano didn't fit through those small windows so they got stuck just holding him ransom!
Counter-Terrorists: Get out of the waiting line for a Ho, sneak out of the Bordelle, and stop those crack-lit T's from damaging Mr. Friedrich anymore than he already is....he can barely walk. Bring him back to a safe place to wait for backup. Donuts are on their way...
Terrorists: Man....the pigs are coming...and you need more crack..so stop em from diggin into your turf and force them to fork over the cash. If Mr. Friedrich should happen to die, don't worry, they'll make more piano players!!

I tried the maps quoted above, but it works for me, even unlogged.

Also, I don't see anything obvious in the logs at first glance. Hopefully with a recent example, I will see something.

I still can't reproduce it, but the access log shows that the web server request on cs_warheadx.zip was successful. At the same time, the error log shows "AH01071: Got error 'Primary script unknown'." It's unclear why.

The MySQL log is empty, which is odd. So, I updated the server and did a full reboot to generate a new file and reset things.
